Pharmacy Team
The Clinical Pharmacy team are highly qualified experts in medication. Working collaboratively with other healthcare professionals they play a vital role in improving quality of care for our patients. They work closely with each of the five surgeries within the PCN, supporting them in a wide range of medication related areas, including patient safety. The team also offers support to patients through medication reviews. These help patients to better understand their medication, and ensure they are taking the right medication, in the right way, in order to achieve the best outcomes for our patients.
